Evaluation of Platelet Aggregation Test in Patients with Bleeding Tendency


Abstract
Platelet aggregation test (PAT) is an useful tool to detect abnormal platelet function in patients with bleeding tendency. The purpose of this study is to investigate the platelet aggregation pattern in various bleeding disorders and to evaluate
the
efficiency of PAT for detection of thrombotic diseases.
Authors examined 38 patients requested PAT to elicit the cause of bleeding tendency. Aggregometer 500 R (Chronolog Corporation, Havertown, PA, USA) were used for PAT. Aggregating reagents such as ADP, epinephrine, ristocetin, collagen, and
thrombin
were
added to 500 ul of platelet rich plasma (PRP).
Of 14 chronic myeloproliferative diseases (CMPD) patients, 10(71.4%) showed no aggregating response to epinephrine, 6(42.8) revealed no response to ADP. There was no secondary aggregating wave in 1 patient with chronic osteomyelitis. One case
with
liver
cirrhosis showed severely decreased function against all aggregating reagents. Of 6 patients with thrombosis, no patients showed increased activity of platelet function.
In conclusion, PAT appears to be sensitive for detecting decreased platelet function, however, insensitve for detecting increased platelet function. Therefore, in vivo function test, such as platelet aggregate ratio are recommended as PAT for
thrombotic
patients.
